RCA

Listed some procedure and steps to do RCA for defects. I conducted RCA in my organization by using following techniques

 1.  Collect all defects Which was raised on last release.

 2.  Select defects based on priority or severity  because  we cant do RCA for all defects. Some times user        can  select randomly or priority basis.

 3. Send it all people to analysis the defects , before start discussing on that.

 4. Once Individual analysis is done then call for meeting (Test Project Lead ) to discuss all peoples.

 5. Discuss the issues  by applying  5 Why technique.

 6. Try to group or categorize above defects into some common category.

 7. Publish the discussion notes to every body in the team .

    Once discussion is over then  prepare presentation on above categories . Because multiple defects may be fall under same category, need to notice and what actions need to taken  to overcome in future.  Apply Fish eye diagram or some graphs to compare your previous RCA results.
